Collaborate System Leadership Programme Application Form
This is an application form for the Collaborate programme. This programme has been designed to ignite and cultivate System Leadership capability within the SEL ICS, and to equip participants with the key knowledge, skills, and behaviours and mindsets necessary to succeed as system leaders.

Important information to read before proceeding with the form: Please be aware that at the end of this application form you will be invited to upload a short video about yourself, your motivations, and a question about your experience. Please make sure this video is no longer than 3 minutes long, any footage after 3 minutes will not be watched or considered in your application. This programme is designed to complement not duplicate existing in-organisation, in-place or in-profession leadership programmes.
Key information about the programme
There are 30 available places in the programme. Duration of this programme is 8 months, from March 2024 - October 2024. Time commitment per month is 1-2 days in person and up to a day for individual/group work. Participants will be confirmed by mid-December.
Key Learning Outcomes
While this programme covers a broad range of topics, example learnings outcomes are: 1. Develop in-depth understanding of system transformation through international best practice in healthcare leadership. 2. Build long-term connections with leaders from across the system. 3. Learn how to foster creativity and psychological safety. 4. Develop a greater tolerance for change and ambiguity. 5. Expand understanding of principles underpinning the development of integrated care systems and population health approaches.
Who we are looking for
The focus of the programme is on system leadership and successful applicants make clear their motivation to play a role in the development of this in South East London, and not just a focus on personal leadership development more broadly. The ideal candidates can demonstrate track record of innovative leadership, are energised by leading change at system level, and have proven ability to question and challenge status quo. We will be looking for a drive for innovation, your ability to lead with compassion, a growth mindset and motivation to lead and be a part of the wider SEL system.

GDPR and Consent Information
Once you have completed the application form and submitted your video, it will be reviewed by the SEL ICB Director of HR and OD to be moved to a folder on SharePoint which will have restricted access. Your application form and video will then be reviewed to determine whether you should be offered one of the 30 places in the programme. SEL ICB have been engaging with a third party organisation called Q5 who have been supporting with the development of this programme. Q5 will be provided restricted access to Jotform and the sharepoint to review your submitted video and the non-identifiable sections of the application form for the shortlisting process. Each application form will be allocated a confidential reference number which Q5 will use to identify you and confirm back to the SEL ICB Director of HR and OD whether or not your application has been successful, using that reference number. After submission, should you decide that you no longer wish to be considered for the SEL ICS System Leadership Programme, please contact workforce@selondonics.nhs.uk. We will then ensure that the information you submitted to us is deleted. As a commitment to your privacy, we will always take the necessary steps to ensure that your information is protected and treated securely, as required by GDPR regulations. Any personal details and information you provide to us will be held in accordance with the Data Protection Act 2018 and stored confidentially by SEL ICB for the duration of this project. We will ensure that all data and recordings are kept safe, and all data will be disposed of once we have finished processing it.
